Role Overview:

Cera Care Ltd is seeking a skilled Care Coordinator to manage the delivery of high-quality care services. As a key member of our team, you will be responsible for matching Care Workers with Clients based on their care needs, ensuring efficient and effective service delivery.

Key Responsibilities:

  • Coordinate care services to meet Client needs and preferences
  • Match Care Workers with Clients based on care requirements and proximity
  • Conduct care assessments and reviews to ensure high-quality service delivery
  • Motivate and support Care Workers to deliver exceptional care
  • Collaborate with the Branch Manager to ensure seamless service delivery
  • Monitor and report on service performance and quality

Requirements:

  • NVQ/SVQ 2 or 3 qualification (desirable)
  • Registration with the SSSC (essential)
  • 2 years' experience in care coordination (desirable)
  • Knowledge of CQC/CI/CIW Compliance (essential)
  • Excellent communication and organizational skills
  • Ability to work flexibly and adapt to changing priorities
