Job Title: Care Coordinator
Job Summary:
We are seeking a skilled Care Coordinator to join our team at Cera Care Ltd. As a Care Coordinator, you will play a vital role in ensuring the delivery of high-quality care services to our clients. Your primary responsibility will be to effectively match Care Workers with clients based on their care needs, while also considering factors such as proximity and availability.
Key Responsibilities:
- Manage the delivery of care services to clients, ensuring that care workers are matched to clients based on their care needs.
- Conduct regular care assessments and reviews to ensure that clients receive the best possible care.
- Develop and maintain relationships with care workers, clients, and other stakeholders to ensure effective communication and collaboration.
- Monitor and report on care service delivery, identifying areas for improvement and implementing changes as needed.
- Collaborate with the care team to develop and implement care plans that meet the needs of clients.
- Ensure compliance with relevant regulations and standards, including CQC/CI/CIW.
Requirements:
- NVQ/SVQ 2 or 3 qualification (desirable)
- Registration with the SSSC (Scotland) or SCW (Wales)
- 2 years' experience working as a care coordinator for a domiciliary care agency (desirable)
- Knowledge of CQC/CI/CIW compliance
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility to work varied shifts and respond to changing priorities
