Job Title: Care Coordinator
Job Summary:
We are seeking a skilled Care Coordinator to join our team at Cera Care Ltd. As a Care Coordinator, you will play a vital role in ensuring the delivery of high-quality care services to our clients. Your primary responsibility will be to effectively match Care Workers with clients based on their care needs, while also considering their location and proximity.
Key Responsibilities:
- Manage the delivery of care services to clients, ensuring high-quality care and excellent customer service.
- Conduct thorough assessments of clients' care needs and develop personalized care plans.
- Coordinate with Care Workers to ensure they have the necessary skills and training to meet clients' care needs.
- Monitor and evaluate the quality of care services, making recommendations for improvement.
- Collaborate with the Branch Manager and other team members to achieve business objectives.
Requirements:
- Relevant qualifications, such as NVQ/SVQ 2 or 3, and registration with the SSSC.
- At least 2 years of experience working as a Care Coordinator for a Domiciliary Care Agency.
- Knowledge of CQC/CI/CIW Compliance and experience in conducting care assessments and care reviews.
- Excellent communication and organizational skills, with the ability to work independently and as part of a team.
- Flexibility to work varying schedules and adapt to changing priorities.
