Care Coordinator

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Harmonize Care Ltd. As a Care Coordinator, you will play a vital role in supporting our clients in their own homes and making a positive impact on their lives.
Your primary responsibility will be to coordinate and deliver high-quality care services to our clients, ensuring their needs are met and their wellbeing is enhanced. You will work closely with our care team to develop and implement personalized care plans, and provide leadership and guidance to ensure the smooth delivery of care services.
To be successful in this role, you will need to possess excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team. You will also need to be flexible and able to adapt to changing circumstances, as well as have a strong commitment to delivering exceptional customer service.
Key Responsibilities- Coordinate and deliver care services to clients in their own homes
- Develop and implement personalized care plans in collaboration with the care team
- Provide leadership and guidance to ensure the smooth delivery of care services
- Work closely with other healthcare professionals and social care teams to ensure seamless care delivery
- Ensure that all care services are delivered in accordance with current best practices and regulatory requirements
- Monitor and evaluate the effectiveness of care services and make recommendations for improvement
- Hold an NVQ level 3 or equivalent qualification
- Have at least 2 years of experience in a senior care role
- Have knowledge of Access Care planning/People Planner or be willing to learn
- Be able to provide leadership and guidance to care staff
- Have excellent communication and interpersonal skills
- Be flexible and able to adapt to changing circumstances
- Have a strong commitment to delivering exceptional customer service
