About the Role

We are seeking a skilled Community Psychiatric Nurse to join our team at Sussex Partnership NHS Foundation Trust. As a Community Psychiatric Nurse, you will work in partnership with service users, their families, and carers to provide high-quality care and support.

Key Responsibilities

  • Undertake assessments of individuals and their families with complex health presentations, including those presenting with higher levels of risk.
  • Develop and implement care plans in collaboration with service users and carers, ensuring that clinical practice is evidence-based and consistent with relevant NICE guidelines and Trust policies.
  • Provide specialist nursing advice and carry out specialist nursing procedures as required.
  • Work collaboratively with multidisciplinary teams to deliver interventions and support service users in maintaining their independence and well-being.

Requirements

  • Registered Nurse with current registration in mental health or a related field.
  • Significant post-qualifying experience in mental health nursing, with a focus on community care.
  • Experience of working with individuals with complex health needs, including those presenting with higher levels of risk.
  • Ability to work autonomously and as part of a team, with excellent communication and interpersonal skills.
