About the Role

We are seeking a skilled Community Mental Health Specialist to join our team at Sussex Partnership NHS Foundation Trust. As a Community Mental Health Specialist, you will work in partnership with service users, families, and carers to develop and implement care plans that promote recovery and well-being.

Main Responsibilities

  • Undertake assessments of individuals and their families with complex health presentations, including those presenting with higher levels of risk.
  • Develop and implement care plans that are tailored to the needs of each service user, ensuring that they are safe and well at home.
  • Work collaboratively with multidisciplinary teams to deliver interventions that promote recovery and well-being.
  • Provide specialist nursing advice and carry out specialist nursing procedures as required.

Requirements

  • Registered Nurse with current registration in mental health or a related field.
  • Significant post-qualifying experience in mental health or a related field.
  • Experience of working with individuals with complex health presentations, including those presenting with higher levels of risk.
  • Ability to work autonomously and as part of a multidisciplinary team.
