About the Role

We are seeking a skilled and compassionate Community Mental Health Nurse to join our team at Sussex Partnership NHS Foundation Trust. As a key member of our multidisciplinary team, you will work closely with service users, families, and carers to provide high-quality care and support.

Key Responsibilities
  • Conduct comprehensive assessments of individuals and their families, identifying complex health needs and developing personalized care plans.
  • Deliver evidence-based interventions, including education, care planning, and discharge planning, to promote recovery and well-being.
  • Work collaboratively with carers, families, and partner agencies to facilitate care and support.
  • Provide specialist nursing advice and carry out specialist nursing procedures as required.
  • Contribute to the development and maintenance of clinical practice, ensuring consistency with NICE guidelines and Trust policies.
  • Supervise junior staff and trainees, promoting professional development and adherence to the NMC code of conduct.
Person Specification
  • Registered Nurse with current registration in mental health or a related field.
  • Completion of Mentorship Course or equivalent.
  • Evidence of continuous professional development.
  • Significant post-qualifying experience in mental health or a related field.
  • Experience of working with complex health problems, including risk assessments and management.
  • Experience of supervising staff and providing clinical guidance.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with diverse stakeholders.
