Job Overview

We are seeking a skilled and compassionate nurse to join our community care team. As a district nurse, you will provide high-quality care to patients in their own homes, working closely with the multidisciplinary team to deliver complex care and support.

Main Duties

Key responsibilities include:

  • Visiting patients in their homes to provide clinical nursing activities and support
  • Working with the wider team to plan and deliver complex care, reducing hospital admissions and facilitating discharge
  • Providing clinical holistic assessment and treatment, promoting optimal levels of independence and wellbeing for patients
  • Facilitating teaching and support for formal and informal carers

Person Specification

Essential criteria include:

  • Registered General Nurse (adult)
  • Educated to degree level or equivalent
  • Knowledge around long term conditions and their management
  • Evidence of ongoing professional development

Desirable criteria include:

  • Previous community services experience

Technical Expertise/Competence

Essential criteria include:

  • Demonstrates an understanding of local and national issues influencing the delivery and development of healthcare
  • Demonstrates a knowledge and understanding of evidence-based care
  • Demonstrates an understanding of the Clinical Governance requirements
