Job Description:

We are seeking a highly motivated Discharge Coordinator Specialist to join our Discharge to Assess, Triage and Response Team. In this role, you will work with service users, families, and carers who are currently in hospital, supporting their safe discharge and providing relevant aftercare and support.

Key Responsibilities:

  • Supporting service users and their families/carers in hospital to ensure safe discharge
  • Maintaining and updating information systems and databases to ensure accurate service transactions
  • Providing advice to customers, contacts, external organizations, and colleagues on policies, procedures, and processes

Requirements:

To be successful in this role, you will need to be passionate about person-centered social care, have a proven talent for involving people in decision-making, and be able to build on their strengths. You will also need to demonstrate excellent communication and interpersonal skills, as well as the ability to adapt your interventions in response to different individuals and situations.
