Discharge Assessment Specialist

Bristol City Council seeks a qualified Social Worker to join the Discharge to Assess team. This role involves working in coordination with health colleagues to provide timely and appropriate support for patients transitioning from hospital care.
About the Role
As a Discharge to Assess Social Worker, you will be responsible for completing assessments, developing comprehensive discharge plans, and collaborating with medical staff to ensure seamless patient care transitions. You will work in coordination with health colleagues to provide strengths-based support and enable service users to leave the hospital in a timely manner.
Requirements
This position requires a recognised Social Work qualification and registration with Social Work England. Experience of working within a Qualified Social Worker - Hospital Discharge team is preferred. Additionally, you must have HCPC Registration, 1 year post-qualifying experience within the UK Social Care industry, and Eligibility to work in the UK.
Salary and Benefits
