Hoop Social Work is seeking a highly skilled and experienced Social Work Professional to join our team as an Adult Hospital Discharge Social Worker. This is a challenging yet rewarding role that requires strong assessment skills and the ability to work in a fast-paced environment.

About the Position

You will be responsible for assessing individuals on hospital wards, completing assessments in line with the Social Services and Well-being Act, and identifying triggers for CHC. You will also work closely with the multi-disciplinary team within the hospital.

The ideal candidate will have a minimum of 6 months experience working in the UK as a registered social worker, a relevant Social Work Qualification, and registration with Social Care Wales/Social Work England. You will also need to hold an Enhanced DBS, registered on the update service or be happy to apply for a DBS, and have 3 years of referencing.
