Care Coordinator Specialist

The Care Coordinator role is a key position within the Primary Care Network, responsible for providing high-quality care to patients. This involves working closely with GPs, other primary care professionals, and community services to ensure seamless patient care.
Main Responsibilities:
- To work in a patient-facing role, identifying and managing a caseload of patients, collaborating with GPs and other primary care professionals.
- To assist with summarising and coding patients' records, ensuring accuracy and compliance.
- To provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers and other primary care professionals.
- To support patients in utilising decision aids and creating single personalised care and support plans, in line with best practice.
About the Role:
- To help people manage their needs by answering queries, making and managing appointments, and providing good quality written or verbal information to aid patient choice.
- Assist individuals to access self-management education courses, peer support, or interventions that promote health and wellbeing.
- To support the coordination and delivery of Multi-Disciplinary Teams within the Primary Care Network, working collaboratively with the Collaborative Care Team.
- Provide an interface between primary care and other services, facilitating smooth transitions for patients.
- To remain aware of statutory and local clinical protection procedures, including systems of referral.
Requirements:
- A commitment to maintaining high professional standards of care and staying up-to-date with relevant aspects of care.
- To have a written professional development plan in place.
- As part of this role, undertake appropriate training as set out by the Personalised Care Institute.
- An awareness of common acute and long-term conditions likely to be seen in general practice.
Salary: £12 per hour, negotiable based on qualifications and experience.
