Care Coordination Specialist

We are looking for a skilled Care Coordination Specialist to join our team at Nurse Plus. As a key member of our homecare team, you will play a vital role in ensuring the highest standards of care for our clients in the Norwich area.
Salary: £30,000 - £35,000 per annum, depending on experience
In this role, you will be responsible for coordinating care plans, communicating effectively with clients and their families, and supervising care staff. You will also be required to maintain accurate records and documentation, and contribute to the development of care strategies that promote optimal patient outcomes.
Key Responsibilities:
- Develop and implement care plans that meet the unique needs of each client
- Communicate effectively with clients, families, and healthcare professionals to ensure seamless care delivery
- Supervise and train care staff to maintain high standards of care and promote teamwork
- Maintain accurate records and documentation to meet regulatory requirements
Requirements:
- Experience in a care coordination role, preferably in a homecare setting
- Strong communication and interpersonal skills, with the ability to build relationships with clients and staff
