About the Role

We are seeking a skilled and compassionate Community GP to join our team at North East Derbyshire Primary Care Network Limited. As a Community GP, you will play a vital role in providing high-quality care to our housebound patients and those in care or residential facilities across our network.

Key Responsibilities

  • Develop continuity relationships with our housebound patients, taking a holistic approach to their management and liaising with other health and social care organisations to improve their care.
  • Provide day-to-day clinical leadership for our multi-disciplinary team, including debriefing after visits if needed.
  • Home visiting for clinically complex cases, end of life care, diagnostic uncertainty, and complex prescribing decisions.
  • Senior clinical triage for the Acute Home Visiting service.
  • Build relationships across the extended team, including social care, ambulance, social prescribers, community, and mental health clinicians.
  • Opportunity for involvement in service development as the team expands.
  • Participate in MDT meetings and provide holistic comprehensive assessments and enhanced advance planning.
  • Maximise best care in the patient's own homes to reduce the need for hospital or care home admissions.
  • Provide end of life care in line with the patient's wishes.
  • Contribute to quality improvement activity and opportunities for teaching and mentorship.
