Heart Failure Specialist Nurse

Job Summary
We are seeking a highly motivated and experienced nurse to join our heart failure service in Derbyshire. As a Heart Failure Nurse/Heart Failure Specialist Nurse, you will play a crucial role in delivering high-quality care to patients with heart failure.
Main Responsibilities
The successful candidate will be responsible for managing a caseload of heart failure patients, ensuring their treatment is optimized to give them the best possible quality of life. You will work closely with local community services, GPs, cardio-renal services in secondary care, and our tertiary providers to provide comprehensive care to patients.

Requirements
- Completed local DCHS and heart failure service induction programmes
- Completed degree-level modules of an approved heart failure course
- Completed a V300 Independent non-medical prescriber programme
- Demonstrate ongoing competence and confidence in managing and leading the care in complex heart failure patients
