The Role

We are seeking a highly motivated and caring individual to join our team as a Care Team Leader. As a Care Team Leader, you will be responsible for leading and supervising your shift, case managing residents, and liaising with families and healthcare professionals to ensure the best care plans are created and delivered.

Key Responsibilities

  1. Lead and supervise your shift, ensuring that residents receive high-quality care and support.
  2. Case manage residents, working closely with families and healthcare professionals to develop and implement individual care plans.
  3. Administer and record medication, ensuring that residents receive the correct medication at the right time.
  4. Carry out risk assessments, identifying potential risks and taking steps to mitigate them.
  5. Work closely with care assistants to support residents with daily needs, promoting independence and dignity.
  6. Develop and implement services, working closely with the leadership team to ensure that our services meet the needs of our residents.

Requirements

  1. NVQ Level 3 in health and social care or equivalent experience.
  2. Proven ability to lead and supervise others, with a strong focus on teamwork and collaboration.
  3. Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and healthcare professionals.
  4. Ability to work flexibly, including evenings, weekends, and bank holidays.
  5. Commitment to delivering high-quality care and support, with a focus on resident-centered care.
