About the Role

We are seeking a highly motivated and organized Central Intake Coordinator to join our team at VON Canada (Ontario). As a Central Intake Coordinator, you will serve as the first point of contact for clients, caregivers, and referral services. Your primary responsibility will be to assist stakeholders in understanding, accessing, and navigating available services and supports.

In this role, you will schedule service delivery for different community support services and work collaboratively with team members and community support partner agencies to maintain up-to-date services, information, and data. You will also communicate with funders, clients, and care teams to coordinate program staff and process referrals, initiate services, implement service delivery changes, and address/respond to phone inquiries from the community, customers, clients, doctors, service providers, and other agencies.

You will be responsible for receiving and coordinating all standardized documentation, data collection, and triaging of calls. Additionally, you will assess clients for service needs, monitor service delivery, and contribute to the development, evaluation, and promotion of the program.

The ideal candidate will have 2-3 years of related experience in community and health-based care, with a strong knowledge of education, facilitation, and proficiency in training groups and/or individuals. You will also possess a proven ability to build strong relationships within the community and demonstrate excellent communication and interpersonal skills.
