As a Support Coordinator with Home Group, you will play a vital role in empowering individuals with complex needs to live independently in the community. You will be part of a passionate team that truly cares and is dedicated to making a positive impact on people's lives.

About the Role

As a Support Coordinator, you will be responsible for creating personalized support plans with your customers, working collaboratively with colleagues to coordinate our small team of Support Workers. You will help customers with daily living activities, such as making meals, shopping, budgeting, and maintaining their tenancy. You will also carry out risk assessments, support planning, goal setting, and regular reviews.

Requirements

To be successful as a Support Coordinator with Home Group, you will need:
  • Passion to support our customers to live their best life, working collaboratively with an eye for detail
  • Experience of creating person-centred support plans and supporting colleagues in working to the plans
  • Experience of coordinating and assessing customer referrals
  • The ability to work on your own initiative, remain calm under pressure, and have a resilient approach
  • Access to a vehicle, with the option to use your own car and receive mileage reimbursement
