Employment Type: Permanent, full-time (37.5 hours per week)
Compensation: £12.61 to £13.36 per hour (£24,664 to £26,123 annually) along with excellent benefits including a Health Cash Plan.
Your Role:
In this rewarding position, you will be at the forefront of supporting our clients, helping them develop essential life skills and achieve their personal goals. Your contributions will significantly impact their journey towards independence.
Key Responsibilities:
- Developing personalized support plans in collaboration with clients and coordinating with fellow Support Coordinators to ensure effective implementation.
- Addressing various client needs, including mental health support, emotional wellbeing, and substance use challenges.
- Assisting clients with daily living tasks such as meal preparation, budgeting, and maintaining their living arrangements.
- Conducting risk assessments, setting goals, and performing regular evaluations of client progress.
- Engaging in meaningful conversations and building rapport with clients, ensuring ample time for collaboration with colleagues.
Your Qualifications:
- A genuine passion for empowering clients to lead fulfilling lives, with a keen attention to detail.
- Experience in creating individualized support plans and guiding team members in their execution.
- Proficiency in coordinating and evaluating client referrals.
- Ability to work independently, manage stress effectively, and maintain resilience.
- Access to a vehicle for work-related travel, with mileage reimbursement provided.
