Care Coordinator

Elliott Hall Medical Centre is a well-established NHS GP practice in Harrow, serving over 11,400 registered patients.
Key Responsibilities- Process and signpost patients to the appropriate healthcare professional based on their presenting condition.
- Answer incoming phone calls, transferring calls or dealing with callers' requests appropriately.
- Process patient requests for appointments and initiate contact with patients, team members, and external agencies.
- Enter read-code data on Emis and photocopy documentation as required.
- Manage data entry of permanent and temporary registrations, patient information, and changes to patient registration.
- Direct requests for information, such as SAR, insurance, and solicitors' letters, to the appropriate team.
- Manage queries efficiently and politely, and carry out system searches as requested.
- Maintain a clean and tidy working area, monitor and maintain the reception area and noticeboards, and support clinical staff with general tasks.
- Participate in practice audit as directed by the audit lead and support administrative staff during staff absences.
- Action incoming emails and scan patient-related documentation, attaching scanned documents to patients' healthcare records.
- Complete opening and closing procedures in accordance with the duty rota and support clinical pharmacist in managing repeat prescriptions.
- Monitor stationery supplies, manage the movement of patient medical records, and attend small group and team meetings.
- Replenish supplies in the office, front desk, kitchen, and consulting rooms as requested, and manage Medical Record Envelopes (MREs).
