About the Role

Elliott Hall Medical Centre is a well-established healthcare provider seeking a skilled Care Navigator to join our team. As a Care Navigator, you will be the first point of contact for patients, visitors, and supporting practice staff.

Key Responsibilities
  • Process and effectively signpost patients to the appropriate healthcare professional based on their presenting condition.
  • Answer incoming calls, transferring calls or dealing with callers' requests appropriately.
  • Process patient requests for appointments.
  • Initiate contact with and respond to requests from patients, team members, and external agencies.
  • Enter read-code data on Emis.
  • Photocopy documentation as required.
  • Data entry of permanent and temporary registrations and relevant patient information as required.
  • Input data into patients' healthcare records as necessary.
  • Direct requests for information, such as SAR, insurance/solicitors' letters, and DVLA forms, to the appropriate team.
  • Manage all queries in an efficient and polite manner.
  • Carry out system searches as requested.
  • Maintain a clean, tidy, effective working area at all times.
  • Monitor and maintain the reception area and noticeboards.
  • Support all clinical staff with general tasks as requested.
  • Participate in practice audit as directed by the audit lead.
  • Support administrative staff, providing cover during staff absences.
  • Action incoming emails when necessary.
  • Scan patient-related documentation and attach scanned documents to patients' healthcare records.
  • Complete opening and closing procedures in accordance with the duty rota.
  • Support clinical pharmacist in the management of repeat prescriptions, ensuring they are processed accurately and efficiently.
  • Monitor stationery supplies.
  • Manage the movement of patient medical records.
  • Attend small group and team meetings.
  • Replenish supplies in office, at front desk, kitchen, and consulting rooms as requested.
  • Medical Record Envelope (MRE) management.
  • Support the surgery.
