Care Navigator

The following are the core responsibilities of the Care Navigator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
a. Processing of incoming and outgoing mail
b. Photocopy / scan documentation as required
c. Input data into the patient’s healthcare records as necessary
d. Answer incoming phone calls, transferring calls or dealing with the callers request appropriately
e. Welcome patients and visitors to the practice, directing requests appropriately
f. Maintain a clean, tidy, effective working area at all times
g. Support all clinical staff with general administrative tasks as requested
h. Manage appointment system
i. Manage rotas for the clinical team and attached staff
j. Manage referrals
k. Keep website up to date
l. Manage information TV screens and keep this up to date
m. IT support (back up tape, dealing with IT issues, printer issues)
n. Stock control of IT consumables
o. Keep the patient leaflets up to date (Practice, complaints, EHMCPA)
p. Keep patient information up to date with health campaigns including waiting rooms
q. Process and distribute incoming mail (document handling)
r. Help to process PMA work
s. Help with timetables for learners and medical students
t. Any other relevant duties to ensure the smooth running of the practice
This job description may need to be changed from time to time as the Practice develops. It is important that all staff are prepared to accept additional duties or relinquish existing ones in order to maintain the efficient running of the practice.
In addition to the primary responsibilities, the administrator may be requested to:
a. Partake in audits as directed by the audit lead
b. Support reception staff, providing cover during staff absences
c. Book appointments
d. Complete opening and closing procedures in accordance with the duty rota
e. Keep staff photo board up to date
f. The processing of changes to patient registration, deduction of record and new patient registration
g. File and store records as required
**Job Types**: Part-time, Permanent
**Salary**: From £10.42 per hour
