Job Summary

We are seeking a part-time Care Navigator to join our friendly and supportive team. As the first point of contact for patients, visitors, and practice staff, you will be responsible for resolving and managing issues, prioritizing your workload, and providing excellent communication skills.

Key Responsibilities
  1. Process and signpost patients to the appropriate healthcare professional based on their presenting condition.
  2. Answer incoming phone calls, transferring calls or dealing with callers' requests appropriately.
  3. Process patient requests for appointments.
  4. Initiate contact with and respond to requests from patients, team members, and external agencies.
  5. Enter read-code data on Emis.
  6. Photocopy documentation as required.
  7. Data entry of permanent and temporary registrations and relevant patient information.
  8. Input data into patients' healthcare records as necessary.
  9. Direct requests for information to the appropriate team.
  10. Manage all queries in an efficient and polite manner.
  11. Carry out system searches as requested.
  12. Maintain a clean, tidy, effective working area at all times.
  13. Monitor and maintain the reception area and noticeboards.
  14. Support all clinical staff with general tasks as requested.
  15. Participate in practice audit as directed by the audit lead.
  16. Support administrative staff, providing cover during staff absences.
  17. Action incoming emails when necessary.
  18. Scan patient-related documentation and attach scanned documents to patients' healthcare records.
  19. Complete opening and closing procedures in accordance with the duty rota.
  20. Support clinical pharmacist in the management of repeat prescriptions.
  21. Monitor stationery supplies.
  22. Manage the movement of patient medical records.
  23. Attend small group and team meetings.
  24. Replenish supplies in office, at front desk, kitchen, and consulting rooms as requested.
  25. Medical Record Envelope (MRE) management.
  26. Support the surgery.
