An exciting opportunity exists for a dedicated and compassionate Band 5 staff nurse to join our multi-professional Intermediate Frailty hospital avoidance service in Bridgend.

This post is based within the Acute Clinical Team, offering frailty crisis response and acute clinical management to patients in their own homes within the community.

We welcome applicants from nurses who are committed to providing high standards of care and are committed to ongoing professional development.

Main duties include:

  • Clinical assessment and monitoring of patients, involving undertaking patient observations, vital signs, weights, and taking venous and capillary blood testing to inform clinical decision-makers on treatments and therapies for acute clinical problems and diseases.
  • Collecting other investigations including non-blood biochemistry and microbiology samples, notably taking Electrocardiograms (ECGs), bladder scans, or 24-hour blood pressure monitoring.
