We are seeking a highly motivated and experienced Registered Mental Health Nurse to join our Community Mental Health Team as a Care Coordinator. The successful candidate will work as part of a multidisciplinary team to provide high-quality community mental health services to individuals with severe mental health problems.
Main Responsibilities- Assess, implement, and evaluate care and treatment plans for individuals on the caseload.
- Work autonomously but under the indirect supervision of a Band 7 registered nurse/team manager.
- Collaborate with unregistered healthcare assistants, other health colleagues, and colleagues from the Local Authority to deliver a supportive service.
- Participate in mental health assessments and provide a point of contact for existing service users in the absence of their care coordinator.
- Support clinic nurses during times of leave and participate in various clinics, including depot, Clozapine, Lithium, and physical health screening clinics.

Requirements- Registered Mental Health Nurse qualification.
- Postgraduate qualification or evidence of working towards the same.
- Experience of working at Band 5 in a multidisciplinary team framework in Acute Adult Mental Health Services.
- Experience of joint working with other agencies/services.
- Relevant Nursing Degree.
- Diploma in relevant Nursing/Community Studies.
- Experience of working with family and carers.
- Ability to work autonomously and communicate effectively with service users.
- Ability to assess risk and respond appropriately using evidence-based literature.
- Knowledge of evidence-based practices within a mental health setting.
- Health Board policies and procedures.
- Medication knowledge.
- Risk assessment skills.
