Care Specialist

**Job Summary**
We are seeking a highly skilled Care Specialist to join our Prison and Adult Social Work Team in South Derbyshire.
The successful candidate will work across the North of South Derbyshire and the Prisons, providing person-centred assessments under The Care Act 2014, and reviews. You will also be responsible for arranging the provision of services to meet eligible needs, working in a person-centred and strengths-based way with clients and their families to maintain and maximise their independence.
Key Responsibilities:
- Carrying out assessments under The Care Act 2014 and reviews
- Arranging the provision of services to meet eligible needs
- Working in a person-centred and strengths-based way with clients and their families
- Undertaking personalised safeguarding enquiries
- Working alongside other departments and agencies to promote multi-disciplinary practice
Requirements:
- Strong communication and organisational skills
- Understanding and experience of safeguarding
- Ability to work in a hybrid environment with the need to travel
