Care and Community Facilitator

**Key Responsibilities**
The Support Coordinator role involves providing outcome-focused support to enable clients to lead valued lives, maintain their tenancies, participate in their local community, and much more.
The ideal candidate will be able to effectively meet the needs of our clients, take additional responsibilities such as rota management, induction of staff, and staff supervision.
They will also work in a flexible manner, which may include working evenings and weekends, participating in our local On Call system, and administering medication.
