Job Summary:
We are seeking a highly skilled Case Manager to join our Community and Social Justice Department. As a key member of our team, you will work collaboratively with community partners to address the complex needs of our participants and provide comprehensive support services.
Key Responsibilities:
- Develop and implement personalized response plans to meet the unique needs of each participant.
- Cultivate strong relationships with community partners to ensure seamless service delivery and referral coordination.
- Maintain accurate records and reports to track participant progress and program outcomes.
- Participate in regular staff meetings and training sessions to enhance skills and stay updated on best practices.
Requirements:
Bachelor's degree or equivalent experience required; 2+ years of relevant experience preferred.
