Responsibilities:

  1. Coordinates and implements individualized care plans tailored to each client's unique needs.
  2. Provides personal care, including bathing, dressing, and grooming.
  3. Offers support and guidance to clients in managing daily tasks and navigating community resources.
  4. Participates in crisis intervention and conflict resolution.
  5. Engages in ongoing training and professional development to enhance knowledge and skills.
  6. Fosters open communication among clients, families, and healthcare providers.
  7. Reports incidents and maintains accurate records.

Requirements:

  • High school diploma or equivalent required.
  • 0-1 year of direct care experience working with individuals with mental health, substance abuse, intellectual disability, or physical health conditions.
  • Effective communication, problem-solving, and critical thinking skills.
  • Ability to work in fast-paced environments and prioritize multiple tasks simultaneously.
  • Valid driver's license and proof of automobile liability insurance required.

About Salary:

The salary range for this position is $25-$35 per hour, commensurate with experience. Additional compensation may be provided based on individual circumstances and negotiations during the hiring process.
