We are seeking a highly skilled and experienced Cognitive Behavioural Therapist to join our team at Pennine Care NHS Foundation Trust. As a key member of our Community Learning Disability Nurse team, you will play a vital role in supporting the delivery of our Young People's Specialist Support Team service.
This is a challenging and rewarding role that requires a high level of expertise in working with children and young people with severe learning disabilities and complex needs. You will work closely with our multi-disciplinary team to provide intensive support and develop person-centred positive behaviour support plans.
Main Responsibilities- Assess and formulate individualised care plans for children and young people with severe learning disabilities and complex needs.
- Develop and implement positive behaviour support plans to promote positive outcomes and reduce restrictive practices.
- Work collaboratively with families, carers, and other professionals to provide comprehensive support and guidance.
- Contribute to the development and implementation of our Young People's Specialist Support Team service, including the creation of policies and procedures.
- Participate in regular team meetings and contribute to the development of the team's strategic plan.
- Relevant health or social care qualification with current professional registration (e.g. R.N.L.D, BSC Hons in Learning Disability Nursing and Social work or equivalent relevant experience).
- NMC registration.
- Post-registration qualifications in related subjects (e.g. Positive Behavioural Support, Community Specialist Practitioner, Autism).
- Completion of CYP IAPT LD.
- Experience supervising therapy assistants.
- Experience of functional assessment and analysis.
- Experience of co-production.
- Experience of facilitating therapeutic groups.
- Understanding of Positive Behavioural Support Principles (PBSP).
- Understanding of government legislation and key policy documents relevant to the role.
- Working knowledge of SEND.
- Understanding of the Transforming Care agenda.
- Understanding of evidence-based practice and the implications for clinical practice.
- Capacity to maintain a high degree of professionalism in the face of highly distressing difficulties and situations.
- Knowledge and experience of working within a Safeguarding Framework.
