About the Role

We are seeking a skilled and dedicated Mental Health Home Treatment Specialist to join our team at Rotherham Doncaster and South Humber NHS Foundation Trust.

As a key member of our Crisis Resolution Home Treatment (CRHT) Team, you will play a vital role in providing high-quality mental health care to individuals in their own homes, promoting recovery and inclusion.

You will be responsible for assessing, planning, implementing, and evaluating care plans for service users, working collaboratively with multidisciplinary teams to ensure seamless care delivery.

A strong focus on service user-centered care, with an emphasis on empowering individuals to take control of their mental health journey.

Key responsibilities include:

  • Providing urgent and emergency triage, advice, and assessment for people in mental health crisis;
  • Undertaking home treatment visits, delivering highly skilled nursing care, including full mental health assessments, care planning, and care delivery;
  • Collaborating with the crisis house team to lead on care for admissions as a service;
  • Facilitating early discharge from inpatient wards where clinically indicated;
  • Delivering home-based treatment interventions, ensuring best practice is implemented;
  • Participating in Mental Health Act assessments, exploring alternatives to formal hospital admission.

In return, we offer a competitive salary of approximately £40,000 per annum, depending on experience, plus a comprehensive benefits package.
