About the RoleThe successful candidate will be responsible for designing and delivering personalized care plans, managing multidisciplinary teams, and providing ongoing support and assessment to clients. A supportive management style and up-to-date experience of tracheostomy care and ventilators within the last 2 years are essential requirements.
Responsibilities- Develop and implement tailored care plans that meet the unique needs of each client
- Lead and manage multidisciplinary teams to achieve outstanding patient outcomes
- Monitor and assess clients' physical and physiological needs, providing timely interventions as necessary
- Deliver high-quality nursing care, including medicine management
- Maintain strong relationships with clients' families and advocate for their needs
- Work collaboratively with community teams, CHC nurses, and CCG commissioners to develop bespoke packages of care
- RSCN qualification with a minimum of 6 months' experience in the UK and an active NMC PIN
- Supportive management style that brings out the best in others
- Up-to-date experience of tracheostomy care and ventilators within the last 2 years
- Advanced communication skills for effective liaison with patients, families, and multidisciplinary teams
- Suitable for Band 5/6 nurses with evidence of effective leadership and management of HCAs
- Full UK driving license desirable but not essential
- Competitive salary of up to £50,000 per annum, depending on experience
- £3,000 car allowance
- Flexible working arrangement: 2 days working from home and 3 days in the community
- Monday to Friday (9-5) hours
- 33 days holiday
