Community Rehabilitation Lead

About the ServiceThe Short Term Assessment, Rehabilitation and Reablement Service (STARRS) provides comprehensive care to patients in their own homes, promoting independence and quality of life. Our service includes specialist assessment, equipment, and short-term packages of care.
Job DescriptionAs a Senior Physiotherapist, you will be responsible for:
- Assessing, rehabilitating, and reabling patients with complex needs.
- Working independently within professional guidelines, Codes of Practice, and organisational policies to deliver high-quality patient care.
- Providing evidence-based nursing care to patients in their own homes, working with agreed management care plans and protocols.
- Using modern technology to monitor patients remotely, capturing clinical data and following up on appropriate clinical action as required.
We offer a competitive salary of £70,000 - £90,000 per annum, depending on experience, plus a range of benefits including pension scheme, life assurance, and annual leave.
As a Senior Physiotherapist at London North West University Healthcare NHS Trust, you will have the opportunity to develop your leadership skills, working closely with our multidisciplinary team to deliver exceptional patient care.
Requirements- Relevant degree with professional registration.
- Evidence of leadership courses/training desirable.
