Community Care Coordinator

We are looking for a highly skilled Community Care Coordinator to join our team in North Manchester.
This role involves coordinating care services for individuals referred to the Community Mental Health Team (CMHT). As a coordinator, you will be responsible for ensuring seamless transitions between services and supporting service users through the assessment process.
