We are seeking an experienced and skilled Clinical Care Coordinator to join our team in Trafford, providing exceptional healthcare services to our patients.

About the Role

The successful candidate will work closely with our multidisciplinary teams, ensuring quality care delivery to adults in their own homes and residential settings. This is a full-time permanent position, offering a competitive salary of £43,141 per annum, plus benefits.

Key Responsibilities
  • To lead and coordinate nursing teams in delivering complex nursing care to adults in their own homes and community settings
  • To work collaboratively with the MDT (Social Workers, GPs, and Allied Health Professionals) to ensure effective patient care coordination
  • To develop and implement clinical skills, as well as participate in training opportunities for personal development
  • To possess a genuine desire to develop both the service and oneself

This role requires the ability to travel throughout different locations and be flexible to meet the needs of the service. A full driving licence and access to a car are essential. The successful candidate will hold a relevant CSP qualification, possess experience working within other areas of nursing, and have evidence of participation in clinical audit.

Main Duties Include:

  • Working closely with the Band 7 team leader to ensure quality care delivery to patients in their own homes and residential settings
  • Delivering patient-centred care, focusing on pride, respect, empathy, consideration, compassion, and dignity
