Community Nurse

We are seeking a skilled and compassionate Community Nurse to join our Urgent Care Service in Thanet. As a Community Nurse, you will play a vital role in supporting the nursing needs of housebound patients living in and around the areas of Thanet.
Key Responsibilities- Provide high-quality nursing care to patients in their own homes, promoting independence and empowering them to self-care where possible.
- Work collaboratively with other healthcare professionals to ensure seamless care delivery and effective communication.
- Conduct thorough assessments and develop individualized care plans to meet the unique needs of each patient.
- Administer medications, perform wound care, and provide symptom control and catheter care as needed.
- Take blood and administer injectable medication, ensuring accurate and safe practice.
