Job Overview

We are seeking a skilled and compassionate Community Nurse to join our Tunbridge Wells community team. As a key member of our team, you will work closely with patients and families to provide high-quality, patient-centered care.

Main Duties of the Job

As a Community Nurse, you will be responsible for:

  • Providing comprehensive nursing care to patients in their homes and community settings
  • Working collaboratively with other healthcare professionals to develop and implement individualized care plans
  • Conducting assessments and evaluations to identify patient needs and develop strategies for care
  • Providing education and support to patients and families to promote health and well-being
  • Participating in quality improvement initiatives to enhance patient care and outcomes

Person Specification

We are looking for a qualified and experienced Community Nurse who:

  • Has a degree in a health-related subject or equivalent qualification
  • Has recent experience of community or acute healthcare gained through education or work experience placement or previous employment
  • Has a strong understanding of professional issues and recent developments in the NHS
  • Has excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and other healthcare professionals
  • Is able to work independently and as part of a team, with a flexible and adaptable approach to work
