About the Role



We are seeking to appoint a motivated and dedicated Community Nurse to join our Urgent Care service in Thanet. As a Community Nurse, you will play a vital role in supporting the nursing needs of housebound patients living in and around the Thanet area.



Your key responsibilities will include enabling and empowering patients to self-care where possible, to stay safe and well at home or in their chosen environment. You will provide valuable care, supporting people to manage their long-term conditions and maximise their independence, which in turn can prevent admission into hospital or enable a quicker discharge if they are admitted.



As a service, we provide wound management, palliative and end of life care, symptom control, and catheter care. You will also take blood and administer injectable medication.



You will work as part of a multidisciplinary team, managing patients with complex needs in the community. You will have the freedom to plan care in your own way, with time to develop your assessment and treatment skills, and access to great supervision and a clinical lead to make sure you're on the right track.

Key Responsibilities



The successful candidate will have:




  • Experience of recent community/acute healthcare gained through education or work experience placement or previous employment.
  • Awareness of professional issues and recent developments in the NHS. Knowledge of up to date clinical practice.
  • Sound communication and interpersonal skills (verbal and written). Ability to organise own work. Ability to cope with change. Ability to use own initiative.
  • Able to support the implementation of local and national agenda for health under the guidance of the team lead/ward manager. Ability to prioritise care delivery in a professional manner, taking into account Trust policies and procedures.

Person Specification



The successful candidate will:




  • Be a Registered Healthcare Professional with relevant Professional body (1st Level Nurse/NMC). Degree in a health relevant subject or willingness to complete a degree (within 3 years of appointment).
  • Have a mentoring qualification equivalent.
