Community Health Nurse

We are seeking a highly skilled and compassionate Community Nurse to join our team in Thanet. As a Community Nurse, you will play a vital role in providing high-quality care to patients in their own homes, promoting independence and improving health outcomes.
Key Responsibilities- Provide comprehensive nursing care to patients in their own homes, including wound management, palliative care, and symptom control.
- Work collaboratively with other healthcare professionals to develop and implement individualized care plans.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
- Provide education and support to patients and their families to promote self-care and independence.
- Participate in multidisciplinary team meetings to discuss patient care and share best practices.
- Registered Healthcare Professional with relevant Professional body (1st Level Nurse/ NMC).
- Experience of recent community/acute healthcare gained through education or work experience placement or previous employment.
- Awareness of professional issues and recent developments in the NHS. Knowledge of up-to-date clinical practice.
- Excellent communication and interpersonal skills (verbal and written). Ability to organise own work. Ability to cope with change. Ability to use own initiative.
- Driving licence valid in the UK and access to a vehicle.
