We are looking for a skilled Social Worker to join our team, providing specialist support to vulnerable adults. As part of our Community Mental Health Team, you will play a critical role in promoting the wellbeing of those we support.
Key responsibilities include assessing service users, developing support plans, and working collaboratively with other professionals to achieve positive outcomes.
Key Performance Indicators:
- Timely completion of assessments and support plans
- Effective case management and coordination
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
