Job Opportunity: Part-Time Social Worker

About the Role:

This is an exciting opportunity for an experienced social worker to support the opening of Building 8b, Wāhi Tautoko (previously Tupuna). The successful candidate will be part of a dynamic team that provides high-quality care and support to individuals with complex needs.

  • The role involves working closely with patients who have an identified discharge pathway on admission but require additional assessment and upskilling to safely transition into the community.
  • The ideal candidate will possess excellent communication and interpersonal skills, with the ability to provide extended assessment, treatment, and rehabilitation following an admission to the adult acute service.
  • The position requires a strong focus on teaching skills to support illness management and help consumers live their desired life. Additionally, the candidate must be able to support integration into the community and develop strong relationships with community providers.

Essential Requirements:
