Care and Support Coordinator

Omni Healthcare is seeking a highly motivated and compassionate Care and Support Coordinator to join our team. As a key member of our team, you will be responsible for providing person-centred care and support to residents, including personal care, mealtimes, and activities.
You will work closely with other healthcare professionals to develop and implement individualised care plans that meet the unique needs of each resident.
