Advanced Specialist Mental Health Pharmacist – Community Rehabilitation Enhanced Support Team (CREST)

Berkshire Healthcare NHS Foundation Trust is seeking an enthusiastic and self-motivated pharmacist with experience in mental health to further develop their skills and knowledge in the specialist area. The successful candidate will support our Community Rehabilitation Enhanced Support Team (CREST), which aims to provide a holistic, multidisciplinary rehabilitation function for patients with complex Psychosis and severely impaired social and everyday functioning.

Main Responsibilities

  • Provide high-quality, specialist clinical pharmacy service to CREST, ensuring safe, clinically effective, and cost-efficient use of drugs.
  • Work as part of the multidisciplinary team to develop understanding of the needs of patients with complex psychosis in the CREST service.
  • Ensure best practice in medicines optimisation.
  • Deliver on clinical advice and audit.
  • Provide clinical pharmacy advice on the management of patients with mental illness to maximise benefit and minimise risk in relation to medicines.
  • Deliver education and training on mental health pharmaceutical issues to various health and social care professionals within the CREST service model, the wider integrated care system, primary care, and external stakeholders.
