Job Title: Clinical Pharmacist - Mental Health Community Rehabilitation Enhanced Support Team

Job Summary:

We are seeking a highly skilled Clinical Pharmacist to join our Community Rehabilitation Enhanced Support Team (CREST) in Berkshire Healthcare NHS Foundation Trust. As a Clinical Pharmacist, you will play a vital role in providing high-quality clinical pharmacy services to patients with complex psychosis, ensuring safe, clinically effective, and cost-efficient use of medications.

Main Responsibilities:

  • Provide high-quality clinical pharmacy services to CREST, ensuring safe and effective use of medications.
  • Work collaboratively with the multidisciplinary team to develop understanding of patient needs and provide clinically appropriate interventions.
  • Ensure best practice in medicines optimisation and deliver on clinical advice and audit.
  • Provide clinical pharmacy advice on patient management to maximise benefit and minimise risk.
  • Deliver education and training on mental health pharmaceutical issues to healthcare professionals.

Requirements:

  • Good Pharmacy Honours degree (BPharm or MPharm)
  • Clinical or psychiatric postgraduate certificate
  • Registration with the General Pharmaceutical Council (GPhC)
  • Substantial post-registration experience as a practising pharmacist
  • Excellent working knowledge of mental health therapeutics and prescribing issues
