Home Group is seeking a highly skilled and compassionate Support Coordinator to join our North Tyneside Transitions team.
About the Role
As a Support Coordinator, you will play a vital role in supporting our customers with complex needs to develop their skills and live more independently in the community.
Key Responsibilities
- Supporting customers to manage their tenancies and prepare for independent living.
- Carrying out risk assessments, support planning, goal setting, and regular reviews.
- Visiting customers to conduct support sessions through the Home Achievement Program.
- Collaborating with colleagues to ensure effective support delivery.
Requirements
To be successful in this role, you will need:
- A passion for supporting customers to live their best lives.
- Experience of creating person-centred support plans and supporting colleagues.
- The ability to work on your own initiative and remain calm under pressure.
- A vehicle insured for business purposes and the ability to travel.
