Job Title: Scheme Coordinator

We are seeking a highly motivated and experienced Scheme Coordinator to join our team at Stonewater. As a Scheme Coordinator, you will play a vital role in delivering local services to our customers, creating a vibrant community and sustainable living environment.

Key Responsibilities:
  • Carry out daily well-being checks and respond to emerging changes in circumstances.
  • Identify and manage risk, including safeguarding, and take accountability for ensuring that appropriate actions are taken.
  • Update customer records, in relation to well-being and risk, and manage workloads effectively.
  • Engage with older people and their families, supporting them to identify issues that are important to them and how they would like to progress them.
  • Develop relationships with partner agencies that provide well-being opportunities and support customers to produce and access a programme of well-being and social initiatives.
  • Routinely carry out health & safety, fire safety, and warden call system checks to ensure compliance and high service standards.
Requirements:
  • A strong commitment to delivering a high level of customer service in a fast-paced working environment.
  • Natural relationship building skills, with the ability to motivate and inspire others.
  • Experience of partnership working with specialist agencies for older people, with a passion for co-producing innovative services.
  • Demonstrable knowledge of Adult Safeguarding.
  • Excellent interpersonal skills, with the ability to adapt your approach depending on your audience.
  • Comprehensive understanding of professional boundaries, with a high level of personal integrity.
  • Good general IT skills, including Microsoft Excel and in-house systems.
