Key Responsibilities:
  • Carry out daily well-being checks and respond to emerging changes in circumstances.
  • Identify and manage risk, including safeguarding, and take accountability for ensuring that appropriate actions are taken.
  • Engage with older people and their families to support them in identifying issues and progressing them.
  • Develop relationships with partner agencies to provide well-being opportunities and support customers to access a programme of well-being and social initiatives.
  • Routinely carry out health & safety, fire safety, and warden call system checks to ensure compliance and high service standards.
Requirements:
  • A strong commitment to delivering a high level of customer service in a fast-paced working environment.
  • A natural ability to build relationships and motivate others.
  • Experience of partnership working with specialist agencies for older people, with a passion for co-producing innovative services.
  • A comprehensive understanding of Adult Safeguarding and professional boundaries.
  • Excellent interpersonal skills and the ability to adapt your approach depending on your audience.
  • Good general IT skills, including Microsoft Excel and in-house systems.
