Key Responsibilities:- Carry out daily well-being checks and respond to emerging changes in circumstances.
- Identify and manage risk, including safeguarding, and take accountability for ensuring that appropriate actions are taken.
- Update customer records, in relation to well-being and risk, and manage workloads effectively.
- Engage with older people and their families, supporting them to identify issues and progress them.
- Develop relationships with partner agencies to provide well-being opportunities and support customers to access a programme of well-being and social initiatives.
- Routinely carry out health & safety, fire safety, and warden call system checks to ensure compliance and high service standards.
- A strong commitment to delivering high-quality customer service in a fast-paced environment.
