**Job Title:** Care Leavers Support Specialist
About the Role:
We are seeking a skilled and compassionate Care Leavers Support Specialist to join our team. As a key member of our Care Leavers Hub, you will play a crucial role in supporting young people (16-25) preparing to leave care and transition into adulthood.
Key Responsibilities:
- Manage a caseload of young people and ensure their needs are met.
- Collaborate with partners (Housing, Health, education, training, and employment) to provide a high-quality service to Care Leavers.
- Facilitate stability and safety while helping young people achieve independence at their own pace.
About You:
We are looking for someone who understands the challenges facing Care Leavers and knows how to mitigate adverse life experiences. You will be passionate about improving outcomes for care leavers and work in a fast-paced environment.
