We are seeking a highly skilled and compassionate Mental Health Clinician to join our team at Oxford Health NHS Foundation Trust. As an Integrated Care Specialist, you will play a vital role in delivering high-quality, person-centered care to individuals with severe mental illness.
Main Responsibilities- Support the Clinical Lead in developing and implementing the service, ensuring seamless integration with primary care and community resources.
- Supervise and lead a team of staff, providing guidance and mentorship to ensure excellent patient outcomes.
- Screen, triage, and assess referrals, providing timely and effective interventions to support patients in need.
- Develop and implement care plans, working collaboratively with patients, families, and healthcare professionals to achieve optimal results.
- Map community resources and assets, identifying opportunities for partnership and collaboration to enhance patient care.
- Current professional registration as a Registered Mental Health Nurse or Occupational Therapist.
- Postgraduate education in a relevant field, with a focus on client-centered care and psychosocial interventions.
- Experience working in community mental health services, with a strong understanding of person-centered care and its application.
- Specialist knowledge of psychosocial interventions and their appropriate use, as well as an understanding of the mental health needs of adults.
